custom templates & list of plugins

Hi,

Two questions please:

Where can I outsource a custom header/template? Is it something I can outsource to DP forum or similar?

And where do you have a list of all your plugins and descriptions so I add the up costs prior to possibly ordering? Are these fees yearly, or one-time?

The payment plugins are included in the package.
Integration plugins are generally $40, one time.
