Add Payment keeps giving me date error message

    Hello, I'm trying to add manual payment to one user and I'm keeping getting the message "Date must be in format m/d/y" although my entry date is 12/14/11. Any suggestion?

    On the "Payments" page, the "Add Access Manually " also doesn't seem to work. It doesn't do anything. Please advise.
