Affiliate signup form

Hi
We are trying to set up the affiliate system, kind of next to our member system.
We use compulsory custom fields for our members, which we do not want to use for our affiliates.
When creating the custom fields, you have the option to not "Display in affiliate signup form".

But how and where is the affiliate form?
I can't find any settings or anything in the manual that helps me?

The affiliate signup page is at /amember/aff_signup.php.
