Form Editor: How to add a custom field?

I am trying to add a field (Company Name) with the address information on the "Profile" form.

When I add a "HTML Text" brick, I get a place to put a chunk of code. What do I put in there to simply add a text field like the others (street, phone, etc)?

You will need to first create the field under Configuration -> Add Fields. Then is will be available as a brick in the Form Editor.
