Hello, I'm not sure why, but the fields "Credit Card Number" and "Credit Card Code" don't have the small description underneath that gives a bit more info about the field. Looking at the file default.php I can find the same description for one of the descriptions ( 'for example: 1111-2222-3333-4444' => 'for example: 1111-2222-3333-4444',), however the form doesn't get populated with this information/label. Any suggestion is appreciated. Thanks,
Other thing that I noticed was that the field "State" is mandatory but doesn't show error message when you try to submit the form and it is blank (like the other ones).
